Unlike some email clients, our followups are not just indicated by a flag or a star. They are special reminders that show up on your calendar. Each followup has a comment to remind you why you are followup up along with a link to the thing you are followup up with.

Currently, you can follow up with any of your contacts as well as any item (Goal, Project, Task, or Action) by simply right-clicking (on a desktop) or tap-and-hold on a touch device. This will bring up a special menu where you can choose to followup.
